Moving cat from Washington DC to Lima, Peru

Hello,

I am moving my 8 month old cat to Peru in January and am traveling with Avianca. Does anyone know the process of taking a cat to Peru with Avianca? What forms do I need, do they provide a crate, how much does the whole process cost? Any advice would be great
